The following documents are Adobe Acrobat (.pdf) format. Please click on the link and it will come up on the screen, print it and fill in all of the requested information, including reading and initialing the participation clauses. Please bring the filled in forms to the registration dates (May 1, 8 and 15) at the Warner Park Community Recreation Center or mail it along with the required fees to the address below. Each football player or cheerleader must have a completed football registration form and medical form. If you are going to request a scholarship please fill out the scholarship form and be prepared to present a copy of your most current Federal Tax Return forms for verification of your income.

The online registration system, allows users to use Visa and Mastercard (credit cards and debit cards). All registrations will only be counted as a registration with completed registration form, medical form and $50 minimum deposit.

FEES:

Football Fees: 

First child: $180.00 

Multiple Children in the same family will get a $45 discount for each additional child in football and $20 discount for each additional cheerleader.

Scholarships are available if you qualify. Call for availability and pricing. 

A $15 additional fee is assessed after May 30 for football and cheerleading fees.

Cheerleading Fees:

First Child: $65.00

Scholarships:

Scholarships are only available for kids attending an East High School Area elementary or middle school. The only exception is Whitehorse Middle School as it serves an area that is a borderline area that kids are allowed to attend either east or Lafollette High Schools. In order to qualify, download the scholarship forms and send it in with your registration and medical forms, along with a payment of $100. If your schlolarship is not okayed, a WPYF representative will contact you. We only have 65 scholarships available for the 2012 season, so they will be given on a first come first served basis.
